NOMINATION FORM

Purpose

To recognize employees for outstanding contributions to the college and the community

Criteria

• Employees may be full- or part-time
• Employees must have completed 1-year of employment or 3 quarters of teaching
• Employees are eligible for the award every 3 years

Employee must meet the following criteria addressed in the nomination:

• Demonstrate a positive attitude
• Lead change
• Foster an inclusive workplace
• Work smart
• Respect diversity
• Be service centered

Type and Number of Awards

A maximum of 4 awards are presented annually; 1 award each academic quarter. A certificate and appropriate memento is awarded to each recipient.

Nomination Process

Any employee, student, or community member may nominate a full- or part-time employee. Nominations submitted are kept as active for three consecutive quarters.
