How to Complete a New Form

Electronic Form
- Complete the form;
- Click on "Submit" button at the bottom of the page;
- Form will be electronically sent to the appropriate recipient.
Automated Word Form
- An Automated Word form will have "shaded boxes" in the response areas;
- Open the form;
- Do a "Save As" to your Desktop;
- Open the form on your Desktop;
- Complete the form and save it;
- Open Microsoft Outlook;
- Click on New Mail Message
- Enter Recipient's name and Subject
- Click on "Insert" on Toolbar > File
- Desktop will open, Highlight your File;
- Click on Insert, document will be inserted in your email;
- Send to recipient.
Word Form
- Print out the form, complete and mail it to the Recipient via "Inner Office" mail.
