Form Name: Records Disposition Report - Schedule 151

Instructions: This form is used when destroying records that have met their retention period. The form should be printed out, filled in, signed by the appropriate personnel. A copy should be sent to the Archives for permanent file, with a copy kept in the department as desired. Once completed, send the original form to the State Records Management office at the address shown.
