Form Name: Use of Student Work - Release Form

Instructions: In our syllabi, our "Use of Student Work" policy states that we must get written permission from a student, if we want to use their work beyond the process of regular class instruction. This form is a way for a GCAD (or any Visual Arts) instructor to obtain that written permission.

There is no need to forward this form to our department. Once completed, the instructor should keep it on file as proof of written permission to use the student's work.
