Form Name: Non-Credit Course Information Form

Instructions: This form is for everyone at Metro outside of community education who needs a non-credit course number for either a class or event that their department is sponsoring. They may need to track enrollment and/or charge a fee and pay an instructor. I would like it completed online and submitted to me. The 3 examples at the bottom of the form should explain what information needs to be filled out. I will then enter the course, assign a course number and email the course number back to the requestor.
