Form Name: Determination of Independent Contractor vs Employee Status

Instructions: To comply with IRS guidelines, the College is required to document the appropriateness of paying an individual as an independent contractor or as an employee whenever an individual performing services is retained. Completion of this form will assist staff in determining the most appropriate payment method.
