Form Name: Program Action Request (PAR)

Instructions: The New Program Requirements/Program Revisions form is completed by the Dean's Office when program requirements for new degrees, certificates, or specialty diplomas are established, or when changes to existing program requirements need to be transmitted to Academic Affairs. After the Dean signs it, it is forwarded to the VP for Academic Affairs.
