Form Name: Readmission From Suspension or Dismissal Form
Instructions: Readmission process
Student: 1. Makes appointment to meet with counselor 2. Completes Readmission from Suspension or Dismissal form (available on Forms Bank). 3. Develops plan to improve academic performance (with assistance from counselor). 4. Counselor forwards form to Student Services Administrator for review.
For Readmission from Suspension
Campus Director:
1. Reviews student plan and may meet with student. 2. If readmission is approved, updates students status to Academic Observation ( SACS).** 3. Removes Suspension Hold on students record on Colleague (PERC) and places an Academic Hold (AO). ** 4. Returns form to counselor who worked with student.
Counselor:
1. Notifies student of appeal outcome. 2. Enters plan in Student Remarks (STRK). 3. Scans form and sends electronic copy to Campus Director to file and keep for a time period as determined by each Campus Director. 4. Counselor retains original form for their records.
For Readmission from Dismissal
Camp Deans/Executive Directors:
1. Meets with student and reviews student plan 2. If readmission is approved, updates students status to Academic Observation ( SACS). 3. Removes Suspension Hold on students record on Colleague (PERC) and places an Academic Hold (AO). 4. Returns form to counselor who worked with student
Counselor:
1. Notifies student of appeal outcome. 2. Enters plan in Student Remarks (STRK). 3. Scans form and sends electronic copy to Campus Dean/Exec Director to file 4. Counselor retains original form for their records.
F-1 intl students who were suspended or dismissed must contact the International Enrollment Office to reapply and discuss their immigration statuses.
**All counselors have the rights to make changes in PERC and SACS in the absence of a Student Services Administrator
