Form Name: Drop and Delete Log Sheet
Instructions: This form is to be used by Call Center Agents to log drop/delete transactions while on Call Center duty for Quick Question Advising. The agent should indicate if the transaction is a delete or drop by placing an X in the appropriate box. Document the total number of remaining credit hours the student is registered for after the transaction is completed. At the end of your Call Center shift, fax the completed log to Central Registration at: 402-457-2265.
