Cost: Housing rent is $280.00, monthly deposit is $245.00.

Requirements: Need to have severe and persistent mental illness.

Steps/Process: Need to be referred to the program by a psychiatrist, APRN, Therapist, Nurse, Case manger or a Community support specialst.

Faith-based?: This agency is faith based with a church on site and a chaplin, however this is not mandated.

Requirements:

  1. Must be an active Medicaid recipient age 5 (varies by state) or older.
  2. Can be referred by an agency or self-referred.
  3. Must be willing to take an active part in the treatment process.

Steps/Process:

  1. An intake specialist first performs a medical evaluation.
  2. A licensed therapist meets with the client for a diagnostic assessment and determines whether the client is eligible for services.
  3. A person-centered plan (PCP) that explores the client’s goals is created. If the client is a child, the parent(s) also assist with the PCP.
  4. The PCP is submitted for approval to the state’s authorization department.
  5. Client is assigned to a therapist/qualified professional. The PCP is reviewed periodically to track progress.

Additional Details: The mission at Life Enhancement Services, Inc. is to support and encourage personal growth and independence in developmentally, socially and mentally challenged individuals.

Steps/Process: Call our main number to give information for an intake form and to be placed on the waiting list to see a therapist. If the therapist feels they need to see the psychiatrist that can be scheduled.

Faith-based?: No

Additional Details: We offer Chemical - alcohol and substance evaluations;
IOP - intensive outpatient therapy;
RTA - for adult sex offenders (through our Childrens Service Dept);
R-Safe for adults and children who have been sexually assaulted, abused or act out sexually (through our Children's Services Dept).

Cost: No fee for our services. If a mental health evaluation is needed, we will assist with that process. Region Six provides case management services to assist young adults with transition into the community and achieving independence.

Requirements: Must be ages 16-26.
Must live within the counties of Douglas, Sarpy, Washington, Dodge, and Cass.
Must have a mental health diagnosis or have mental health needs.

Cost: Taylor Family Counseling provides for Medicaid, bluecross/blueshield, Coventry, midlands choice, Aetna and others. Please call to get fee for service charges.

Requirements: There are no predetermined requirements to get mental health services. Taylor Family Counseling is an outpatient mental health program. If it is determined during the initial assessment that in-patient services is more appropriate then the client is referred to an in-patient program.

Steps/Process: In order to access services call (402) 210-8154 and schedule an appointment. Currently there is no waiting period.

Faith-based?: Taylor Family Counseling is not a faith based program however, religious beliefs and practices are incorporated to meet client needs.

Additional Details: Services are provided in a nonthreatening emotionally safe manner with respect for the client's self-determination.
