- You may complete a request form at any student service location (no fee required).
- You may request a transcript online.
- You may mail in or fax your request; please complete and sign the attached form.
- Include all names used while attending Metropolitan Community College (maiden names and/or names before divorce(s)).
- Include your Social Security Number (if missing or incorrect, processing your transcript may be delayed).
- Include your date of birth.
- State the full name and address.(please refer to the above policy concerning official or unofficial transcripts).
- List your present and former address (street, city, state and zip code). This allows the Records Office to update your record.
- List your last date of attendance at Metropolitan Community College if possible (include quarter and year).
- For purposes of contacting you, please include a current day telephone number with your request.
- Allow 5 to 7 business days to process your request.

A transcript requested by a student will be mailed directly to a business, college or university is considered OFFICIAL. Official transcripts are printed on security paper, stamped with an official seal and signed by the Director of Enrollment Management. Official transcripts are not to be hand carried by students.
Transcripts issued directly to a student or to a student's family are distinctly marked "STUDENT COPY". Colleges and Universities do not accept STUDENT COPIES. Businesses and other agencies use their discretion when accepting a student copy transcript as an official document for their particular use.
