Life Events (Benefit Choices)

Life events may affect your employee benefits. Employees are responsible for notifying the Human Resources office within 30 days of a life event to request a change of coverage (i.e., add/remove a dependent), or to request a mid-year election change under a PayFlex flexible spending account.

Below are the steps employees need to initiate following a “life event”:

Family status changes:

Contact HR and make changes within 30 days of the event of family status changes such as: marriage, divorce, birth or adoption of a child, loss of spouse's insurance or death. Change must be consistent with the family status change.

  • Change health/dental election

  • Change PayFlex election

  • Change beneficiaries

  • Change life insurance elections 

  • Submit new W-4 form

  • Update emergency contacts

Coverage for children:

Health Insurance: Contact HR and make changes within 30 days from the date your child turns age 26. Child is eligible to retain coverage for up to 36 months by electing COBRA.

Dependent Life Insurance: Contact HR and make changes within 30 days from the date your child either turns age 19 and is not a full-time student, graduates, gets married or drops out of school.

  • Change health/dental election

  • Apply for COBA continuation coverage within 60 days (health/dental only)

  • Change PayFlex election

  • Change Dependent Life

Employment changes:

Contact HR and make changes within 30 days of the event of employment changes that affect your benefit eligibility: full-time to part-time, non-exempt to exempt, etc.

  • Check with HR regarding possible enrollment, termination or changes in employee benefits

  • Change Retirement contribution

  • Submit new W-4 form

Periodic review:

Employees are encouraged to periodically review benefits choices including beneficiaries, retirement participation level, life insurance, address changes, etc. There are also planned events intended to provide employees with opportunities to assess benefit needs.

  • PayFlex open enrollment

  • Personal meetings with a TIAA-CREF or Fidelity representative (by appointment)

Other events:

Additional information is available through HR in the event of:

  • FMLA

  • Termination

  • Reduction in force

  • Retirement

  • Death
